Key Responsibilities:

Provide chronic hemodialysis care and develop comprehensive nursing care plans, including assessment, planning, implementation, and evaluation.
Collaborate effectively with physicians and other healthcare team members to interpret and adjust patient care plans.
Conduct patient referrals to dieticians, social workers, or vascular access centers as necessary.
Evaluate patients’ lab results and address any concerns or questions regarding dialysis treatments.
Ensure dialysis machines and equipment are maintained in optimal working condition.
Adhere to the facility’s policies and procedures, complying with state and federal regulations.
Deliver age-specific care, including performing all aspects of the hemodialysis procedure as per state licensure requirements.
Perform technical skills related to patient treatment and emergency procedures as part of the competency assessment program.
Document and report all pertinent patient information accurately and clearly.
Participate in patient and family education, promoting a safe environment for all.
